Outbreak Report – Scrub Typhus in Sikkim

Current Situation:

Information was received on 19.08.04 from the pediatrician and the laboratory in charge regarding cases of febrile illness being reported to the STNM Hospital, Gangtok in Sikkim. 15 cases had been admitted since 1 month from the west & south districts with no apparent clustering in time & place. The patients reported were in the 7-12 years age group and presented with high fever of 10-12 days duration, cervical lymphadenopathy, general body ache, pain abdomen and dyspnoea. 3 of these cases also presented with a papular rash. No hemorrhagic manifestations were seen. All patients had enlarged liver and spleen. Involvement of other systems, namely respiratory, renal and CNS was also seen in some of the cases. Two of these cases brought in the terminal stage died. Routine microbiological investigations did not yield any significant results. Patients who were put on higher antibiotics (ceftriaxone & cefotaxime) in the early phase had a shorter course of the illness, while the ones reporting late in illness took more time to recover. 

 

Laboratory Information:

Serum samples were collected from 5 of these cases (average 10-15th day of fever) and sent to NICD, Delhi for further analysis. 3 of these samples have tested positive for IgM antibodies to Scrub typhus while all the samples were negative for IgM to dengue and Leptospira.
