HIV-AIDS

 

Accelerating the scale-up of HIV/AIDS treatment and care

Treatment, care and support:

*      Antiretroviral therapy for children and adults living with HIV/AIDS

*      Prevention and management of opportunistic infections and comorbidities

*      Care, including nutrition, palliative care and end-of-life care

*      Linking HIV/AIDS and tuberculosis services

*      Nutrition and HIV/AIDS

The “3 by 5” initiative, launched by UNAIDS and WHO in 2003, was the global target to provide three million people living with HIV/AIDS in low and middle income countries with life-prolonging antiretroviral treatment (ART) by the end of 2005. It was a step towards the goal of making universal access of HIV/AIDS prevention and treatment accessible for all who need it as a human right.

On November 30th 2003, as part of the World AIDS Day commemoration, the Government of India (GOI) announced a policy to provide free antiretroviral therapy (ART) with implementation starting on 1st April 2004. The GOI plans to provide free ART to 100,000 patients by 2007 and 300,000 by 2011 (NACP-3).

With technical support from WHO, the GOI has scaled-up ART from 8 centers in six high-prevalence states and NCR Delhi to 127 ART centers covering all states. Access to treatment and care is an important component of a comprehensive programme to combat HIV/AIDS, complementing prevention interventions and forming the basis of the Continuum of Care for people living with HIV/AIDS (PLHA). Concurrently, there is increased recognition and work in strengthening the links with the civil society, private, voluntary and intersectoral providers of care and treatment for the delivery of good quality prevention, care and treatment services. These include the following:

*     Development of technical guidelines for HIV/AIDS in adults

*     Development of technical guidelines for Paediatric HIV/AIDS

*     Development of operational guidelines for ART centers

*     Development of the training curriculum in HIV/AIDS management for specialists and medical officers

*     Development of the comprehensive district training package in the HIV continuum of care and support (based on the WHO IMAI package)

*     Development of tools for ART adherence

*     Technical support for the development of the Engender Health training package for treatment literacy for counselors and peer educators

*     Assistance in procurement of ART and supply management including tools for supply management

*     Development of assessment tools for the preparedness of the health facilities to provide ART services

*     Development of the NACO ART programme recording and reporting tools

*     Assistance in identifying needs and gaps in the national programme

*     Assistance to NACO and Clinton Foundation in developing the training curriculum for private medical practitioners

*     Provision of technical assistance for scale-up for the ART programme through a network of technical consultants at central and state level (2004-2006)
